On Hard Doctoring begins in an emergency room at three in the morning: the hour when the pretenses fall away and medicine reveals itself for what it is: an encounter between one human being who is suffering and another who has agreed to help. Over forty years, Mark Brauner has stood at that threshold thousands of times. This book is his account of what he has learned there.
It is a book about presence: the discipline of truly seeing the person in front of you, not the diagnosis, not the chart, not the liability. It is a book about moral injury: the slow accumulation of damage that happens when physicians are asked to do things that violate their deepest sense of what medicine is for. It is a book about the emergency room as a moral space, a place where the stakes are always real and the margin for self-deception is thin.
Brauner writes about AI and what algorithms cannot replace in the clinical encounter. He writes about nursing: the invisible labor that holds medicine together while the system ignores the people doing it. He writes about Gaza, where he has practiced humanitarian medicine at the edge of what is possible. And he writes about what it means to remain human in a profession that rewards efficiency over encounter, throughput over presence, speed over the slow, difficult work of actually being with another person in their suffering.

A Note from the Author
I have been trying to write this book for twenty years. Not because I lacked the material: the emergency room provides more material than any writer could use in a lifetime. But because I was not sure I had earned the right to say what I needed to say. I am not sure I have earned it now. But I have reached the point where the cost of not saying it is higher than the cost of getting it wrong. This book is my attempt to be honest about what medicine is, what it does to the people who practice it, and what it could be if we were willing to demand more of ourselves and of the system we work within.

Dr. Brauner's second book turns from the individual clinical encounter to the system that shapes it. It is a rigorous, unflinching examination of American healthcare: how it came to be this way, why it fails the people it is supposed to serve, and what a genuinely different system would require.
This is not a polemic. It is an evidence-based, solutions-oriented analysis written for readers who want to understand the problem clearly and think seriously about what comes next. Drawing on four decades of practice, research, and humanitarian work, Brauner brings the same moral clarity he brings to the bedside to the structures, incentives, and failures that define American medicine at the system level.
